Internal Medicine: Site Visit Summary

My first site evaluation was done in person with the site evaluator. She spent a long time with me going over my H&Ps, and we discussed the patients in detail. I found this to be helpful because she focused on helping me see what was important to note for a patient admitted vs. outpatient. For example, functional status a patient was admitted, ensuring they have proper evaluation by physical therapy, and then how to ensure that discharge planning was appropriate and safe. We also discussed when to hold certain medications such as blood pressure medications. In the first visit we also reviewed my pharmacology cards and she highlighted important aspects of pharmacology for me to pay attention to when I start prescribing. For example she told me to focus on major drug interactions, and which drugs require renal dosing. 

My second site evaluation was done over zoom, but the same care and discussion was able to occur. We discussed a third patient case and my journal article. The patient case was an elderly man who fell, and who was diagnosed with alcohol use disorder. The case highlighted the importance of recognizing alcohol abuse in the elderly so it can be properly treated to prevent withdrawal and delirium tremens. The journal article I presented related to this case, and it highlighted the importance of screening elderly trauma patients for drug and alcohol use on admission.
